Targeting 15,000 members with open care gaps for colonoscopies (COL), mammograms (BCS), bone density  scans (OMW) and annual wellness visits, the MA plan:

  • Engaged 33% of members called
  • Impacted 36% of care gaps among engaged members
  • Achieved a show rate of 81% at scheduled appointments
  • Earned 9.4/10 stars on ReferWell's member satisfaction surveys
  • Improved data accuracy, updating 21% of member data and 13% of provider data
